چکیده

BACKGROUND Endocarditis may precede or complicate bacterial meningitis, but the incidence and impact of endocarditis in bacterial meningitis are unknown. METHODS AND RESULTS We assessed the incidence and clinical characteristics of patients with meningitis and endocarditis from a nationwide cohort study of adults with community-acquired bacterial meningitis in the Netherlands from 2006 to 2012. Endocarditis was identified in 24 of 1025 episodes (2%) of bacterial meningitis. Cultures yielded Streptococcus pneumoniae in 13 patients, Staphylococcus aureus in 8 patients, and Streptococcus agalactiae, Streptococcus pyogenes, and Streptococcus salivarius in 1 patient each. Clues leading to the diagnosis of endocarditis were cardiac murmurs, persistent or recurrent fever, a history of heart valve disease, and S aureus as the causative pathogen of bacterial meningitis. Treatment consisted of prolonged antibiotic therapy in all patients and surgical valve replacement in 10 patients (42%). Two patients were treated with oral anticoagulants, and both developed life-threatening intracerebral hemorrhage. Systemic (70%) and neurological (54%) complications occurred frequently, leading to a high proportion of patients with unfavorable outcome (63%). Seven of 24 patients (29%) with meningitis and endocarditis died. CONCLUSIONS Endocarditis is an uncommon coexisting condition in bacterial meningitis but is associated with a high rate of unfavorable outcome.

Central nervous system involvement in adult patients with invasive infection caused by Streptococcus agalactiae.

INTRODUCTION Streptococcus agalactiae is frequently an asymptomatic coloniser and a cause of neonatal and puerperal sepsis. Infections in nonpregnant adults are uncommon. The frequency of neurological complications caused by invasive infection with this microorganism in adults remains unknown. Here, we study the frequency and characteristics of central nervous system (CNS) involvement in adults...

Elizabethkingia meningosepticum endocarditis: A rare case and special therapy

Among the Elizabethkingia species, E. meningosepticum is the most pathogenic member of genus. As primarily opportunistic pathogens, they majorly cause meningitis in premature and newborn infants and rarely cause pneumonia, endocarditis, and meningitis in immunocompromised adults (1). Here we describe an adult patient with an E. meningosepticum endocarditis who had no underlying diseases. To the...

Triple valve endocarditis as an unusual complication of bacterial meningitis.

We report a case of Staphylococcus aureus endocarditis with late onset in a 39-year-old male drug abuser, who presented with bacterial meningitis. Despite resolution of the meningitis as the result of appropriate antimicrobial chemotherapy he developed triple valve endocarditis. Some striking features of this case and a comparison with other reported cases of this uncommon presentation of infec...

Arterial cerebrovascular complications in 94 adults with acute bacterial meningitis

INTRODUCTION Intracranial vascular complications are an important complication of acute bacterial meningitis. Ischemic stroke in meningitis is reported as a result of vasculitis, vasospasm, endocarditis or intraarterial thrombosis. The aim of the study was to identify the value of measuring cerebral blood flow velocity (CBFv) on transracranial doppler (TCD) in the identification of patients at ...

Pneumococcal meningitis: antibiotics essential but insufficient.

to the discovery of antibiotics, bacterial meningitis was almost invariably fatal: over 95% of individuals developing bacterial meningitis died, and the few individuals surviving infection were neurologically devastated. With the advent of antibiotics, bacterial meningitis became a treatable condition, and the primary objective in the patient with bacterial meningitis became, appropriately, pro...
